Cancellations or Rescheduling

Prescription refills are best handled at your regularly scheduled office visit. During this time, we are able to review the proper dosages, as well as address any questions or concerns you may have. If it becomes necessary to refill your child's medications between appointments, please allow at least 24 hours for prescriptions to be phoned to pharmacies and at least 48 hours to process requests for written prescriptions.
Chronic or long-term medication refills require an office visit before they may be refilled. Please call us at (801) 479-0174 to let us know what you need.
A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your child's records be sent to another provider we ask that you supply us with their name, address, and phone number.
